Wrong this is a Japanese cold meaning MC has hours to live at most. Although from the many romcom manga I've read, it seems it's not that contagious since the love interest always ends up in close proximity to the afflicted to nurse them and rarely ever gets sick
You're forgetting a common rom-com trope of MC getting better, love interest gets sick as a result, MC feels obligated to nurse them back, they grow closer as a result, common excuse for them getting sick is that MC gave it to them which is why they got better so quickly. Sometimes it's the other way around, depending on who gets sick first.

In Ijiranaide Nagatoro, MC literally gets sick by getting teased into an indirect kiss... By sharing a spoon with the just-bouncing-back-from-a-cold Nagatoro.
