Chapters 11 and 12 have been combined in the volume release. Also all chapter titles have been changed from the first print interestingly.

Yes the content is same far as I know.

Egawa made "Kekkou Kamen R"
Keiko made "Kekkou Kamen '98 beautiful hero" both are some special chapters, released in specific special story collections, don't know any better.
