If I take it seriously, my theories are these:
1) She faked her death and had it planned with Rio before their fight, yeah, even her arm (Almost not possible). The floor she got stomped against wasn't actually solid, it was made of wood.
2) We've seen in KB that you can re-attach a lost limb through a specific medical treatment but it takes time, all I can guess is that Shido or other scientists have been developing a healing s*t to re-attach limbs in less time.
3) All this might be an illusion because of how much blood Shido has lost, and the one who protected him is actually someone else (Hitomi? Maybe?)
4) A mix between all the previous ones.
Well Rio seemed seriously distressed about killing her, which was strange in the first place. Why would she mush her friend / almost lover like that? But she'd be one hell of an actress since she literal threw her fight in part because of that despair. Maybe we get a proper explanation but right now it smells like deus ex monstergirl.

As I've said, the first theory is almost not possible but still a possibility. Who knows?
I can't wait for the next chapter.

Yeah I thought maybe Rio was in on it, but she seemed pretty fucking sure that she killed Mai as she was fighting Pure, so I don't think it was a plan. I think the answer is that the gym floor was hollow underneath and she fell into it. Afterwards the Sons had her stitched back up before sending her back out to go kill Yashin.

My guess is that Mai made a hole in the ground beforehand to fake her death, and beating Rio wasn't her primary goal, like winning Killing Bites/Destroyal wasn't Leo's goal. Maybe she was forced to participate because Shidou was held hostage and she was told to win against Rio, so he was released, but she didn't believe Iwasaki.

Yeah I thought maybe Rio was in on it, but she seemed pretty fucking sure that she killed Mai as she was fighting Pure, so I don't think it was a plan. Who knows? Maybe they had it planned, but the fight was still personal (On Mai's side, at least) with no regrets about the outcome. In KB it seems that even friends don't have a problem fighting each other to death, and some characters here might find disrespectful if you don't fight with all you got.
I agree with the gym floor being hollow as that's the only way Mai's body wouldn't have turned into red paste.

Personally, I think she had as primary goal defeating Rio because of their backstory, as it seemed like it was personal and I think she would have preffered dying fighting, but it seems she just got brutally knocked out.
If she didn't die and that's not another character, she's not in optimal condition to fight Rio again, but she is still Shido's bodyguard.
