@dahhv2 he just said he ain't actually a eunuch(including his advisior/guard)and that
the drug keeps him from hitting that puberty stage(also prevent getting a horny boner). Excessive use in suppressing hormones related to that might cause impotency.

For those who are confused, Jishin is actually 19 (same as our protagonist mao mao) since throughout the manga 2 years have passed, but he is posing as a 24 yr old eunuch as some form of job from his father, the emperor, which is to weed out any unworthy and unfaithful women in the harem (he's the gardener of the emperor's "flower garden"). From what we know, we can assume that Jishin is actually being forced to do this job since he is the only heir to throne atm (a consort's son at that) hence why in this chapter he was really wishing for the emperor to bear a new son. Back then, the only people allowed to stay in close proximity to the women of the harem were other women and eunuchs (b/c they were educated men who were chosen by the emperor to tend to the women of the harem as doctors. They had to be castrated as a precaution for the women of the harem to not be tempted to sleep with these men and vise versa) Jishin, however, is the only heir to the throne, thus it would be a terrible idea if he were castrated and not able to continue the blood line. So, instead of being castrated, he had to ingest medicine that would allow him to appear impotent without actually being impotent.

For the father part, I'm curious as to how you figured out that he is the son of the current emperor (like how were you able to connect it to Jinshin wishing for the emperor to born another son, when Jinshin said child instead of son and since we already saw that there's a baby daughter, so he could have meant that he was hoping for the emperor to born a second child)?

jin shi and gao shun are drinking a medicine made with a ground-up root vegetable .
maybe this root vegetable is the same one in chapter 30 , when the old maid asked mao mao to bring back powder of ground-up taro from the medicine office .

Oh I already suspected that Jinshi is emperor's "brother" because the "brother" was described as sickly and rare to see + him and Aaduo being heavily androgynous* and being directly described as "similar" + them hanging out at her departure, but that just confirms it.
Also it's a shame that he isn't an eunuch, I thought that the humanisation and even the possibility of romance** was nice.
And good lord the pedophilia keeps fucking me up (and harem in itself, and forced castration, and horrors of prostitution, and child body experimentation... there's a lot of upsetting things in this manga, though this doesn't mean that the manga itself is bad. but my heart breaks nevertheless)

*with Aaduo losing womb at a young age and assumption that Jinshi is a castrate, though, it wasn't a strong lead.
**I personally hate his creeping towards Maomao but that's beside the point.

The narrator say he's 24 y old, then just after they say he's the same age as the youngest son, who is 19y old ?

So...while I'm happy so much stuff got explained...why was this chapter named after Gao Shun?
Probably cuz they dropped the most bombs ab gao shuns back ground in one chapter LIKE DAMN
