I actually think given the author final notes that the differing factor is the MC is trans while the friends is not. The friend briefly becomes a magical girl...but its just a tool, a way to a medium, meanwhile the MC changing into a Magical Girls resonates deeper with him as he is a trans woman just have not realized. So it affect him deeper.
