A few thoughts...
1. To prevent those tags being spammed maybe definitions could be added, e.g. when someone tries to add the tag a pop up window will show up with an explanation of the tag and it would ask the editor to confirm that the title meets the criteria.
2. I know of chat platforms where messages are being automatically hidden if reported by at least two users of a certain level/role. Mods can then unhide such messages. So, for faster processing of content warning tags without demanding faster actions from the mods here, MD could auto-tag a title if it was reported by x users. Later, when mods have time to check the series, they could un-tag if needed.
3. Not sure about smut or gore, but I think in case of sexual violence content warnings are important because there are a lot of people who have experienced it and are traumatized by it. And I don't know how widespread the knowledge is but "trigger" used to be a psychological term (before it became a "meme"). If a traumatized person is exposed to such triggers it can lead to severe problems, thus a sexual violence tag is definitely needed and should ideally be put in place in a timely manner to ensure people's safety (yes, "safety". Psychological triggers are not a "meme" and they are not an indicator for "sjw snowflakes" or entitled "Karens"). So rather than questioning the existence of this particular tag itself, I think it would be best to discuss how to make this tag functional for all groups, those readers who are not affected by it and those who are and the mods who can't be present 24/7/365.
