To become a witch, one must have sex with someone. However, we knew that the partner of the sex would not be there after the sex since we saw no witches that have a partner before. Thus, we knew that if they do the deed and perform the ritual, the MC could legitimately die or worse, his body become a vessel for whatever demon that she makes a contract with to become a witch. That's why she said she don't want to involve him with her world and she would find a way for her to become a witch without the sex ritual. Because she genuinely love him, and the thought of using his body and losing him forever is not something that she wants.
The frog thing is probably because she doesn't want to see him for a while, it happened. And meeting him in the dream is a way for her to see if he is serious with his love to her or not, or is he just wanna bang.

Nope, he knew it wasn't her the first time she did it. He didn't know it was a frog, but he very much knew it was wrong.

The reason no one saw the frog is because it's better disguised as compared to last time. The disguise is literally a better wig that looks more like Komori's hairstyle, as compared to the first time the frog was shown with like straw as hair.
