I feel like werewolf (the game) would have been named differently in a world with a real werewolf cause it paints a real group in an unnecessary negative light.

Though us readers won’t be able to tell immediately if the story decided to use a different name (well, using the other alternate name, mafia, would work, but probably not for the Japanese readers).

Based on the first chapter, genjin are an extremely new thing in for human society so I would assume that werewolf was made before then and still has the name

Probably not! The rules of the game are part of why the joke works, much like how the joke wouldn't work if he called it Mafia instead.

I don't remember for sure but I seem to recall there was a lot of bad relations with genjin in this world until recently. Even today in our world there are offensively named games that are relics of the past. For example, in North America there's something called the "game of telephone" but in certain other Anglophone contexts they call it... something else.
