To be fair realizing that he is not meant to be a leader but a soldier is probably the smartest decision he made. Just because someone is a great fighter doesn't mean they're a great leader. Him passing it off to Yumina who is able and willing to get her hands dirty and make the tough decisions is actually pretty smart... he probably should have broadcasted that decision though so everyone will know and not cause confusion.

I think he did in the LN but it was cut out.

It's 100% on Katsuya for not issuing any orders. The chain of command collapsed and people started doing their own thing which lead to a lot of people being in splat range instead of maintaining their relatively safer original plan.

Did Akira ever disobey Alpha once? If he did, then it's his luck that made him survive.

Alpha never made it possible for Akira to "disobey" her. All she does is give suggestions and options. Akira is the cautious type, so it's easier to keep him alive. That said, when Akira does things that Alpha doesn't agree with, she will complain about it. They have good open communication unlike Katsuya.

The only times Alpha takes action not in accordance to Akira's will is when she takes control of vehicles or his power armour to save his life. Might hurt like hell, but better than being dead.

This is why you dont try to show off and follow the plan. The only time to deviate from the plan is if something unexpected happens like a change to the situation or new information comes up that requires a switchup
