I think it's pretty common for Americans who grew up outside of poor cities to view cities as threatening places. There is a lot of cultural propaganda in the USA meant to make working class people hate each other, so they don't end up working together to overthrow the ruling class. It's a class-based bigotry often wrapped up in racial bigotry.

That said unless you grew up in the Philadelphia metro area, or relatively nearby like Princeton or Newark, you probably don't think about Camden specifically, but if Camden were mentioned you would think "Oh it's one of those terrible cities!" And if you grew up far from those cities, you stop separating them in your mind. Camden's ghetto becomes a symbol for Philadelphia, the Bronx becomes your image of all of New York City, etc.

Why did the Author here pick Camden? Maybe because of the Rocky films.

The extra line is added by the mangaka. It's not the most common way to show that a character is speaking in another language, in most cases it's done by making the text horizontal instead of vertical, writing the text in katakana instead of kanji or using a different font.

Probably a combination of Rocky being from Philly and at the time this was written Camden was one of the most dangerous cities in the US. The author wanted a run down dangerous place and Camden was at the top of the list
