Yamada
Cause of death:
Hemorrhagic shock due to rupture of seminal vesicles.
Description of incident:
Hospital staff were called by pt. in same room. Pt. Yamada was found laying supine in hospital bed in decorticate posturing, GCS 1, 2, 3 for a total of 6. Bilateral femur Fx and TBI observed. Ecchymosis found in anteromedial thigh. MOI suspected to be the explosive force of rupture. Nurse Mochizuki, as the senior member of staff on scene, was the first to take charge. Immediately after, Nurse Mochizuki slipped on bodily fluids expelled by the pt., landing on pt. Yamada's chest and causing multiple rib Fx resulting in a flail segment.
Pt. Yamada then began to exhibit dyspnea and HR increased. BP began to rapidly decline, progressing into cardiac arrest several minutes later. Despite hospital staff's best efforts at resuscitating Yamada, TOD was declared at 0148. Nurse Mochizuki has been put on leave and advised to seek critical incident stress debriefing.
