Because they engender a specific response in most people. Even the ones who aren't super bothered by roaches usually find them creepy. And if you think the reactions in manga are silly or too extreme, I've seen many real life women freak the absolute fuck out just by having one come near them.

I'd argue it's the same thing as arachnophobia, but for whatever reason no one ever treats it like a phobia the way they do with spiders.

honestly even i can handle roaches when they just walked around normally, but if they're flying now that's a different story even i will kind of freak off and search any weapon for protection.
