After having re-read this chapter a few times, I think I've come up with an answer to this question that involved
waaaay overthinking a gag manga. The tl;dr answer is "it's complicated". The longer answer is "it depends". The magic is reliant on the caster being a "pure maiden", in other words, a virginal female. With that in mind, Avel could lose their virginity in their male form and still be able to heal with the best of them, since their female body would be unsullied. But that brings up the bigger question of: does "maiden" refer to a woman who's never had sex of any kind, not even so much as a kiss, or just a woman who still has her hymen in tact? If it's the former, then yes, if Avel loses their female body's virginity, the healing power goes away, but if it's the latter, then as long as they stay away from vaginal sex, Avel can get as kinky as they want. However, there's also another factor to consider: Avel's new patron deity is the god of evolution and while we've only seen it manifest as a gender swap, Avel could
theoretically alter their body however they see fit, which could also include returning their body to a virginal state, allowing them to have the best of both worlds.
Now, all that said, since this is a gag manga, I imagine it'll
actually go something like this: Avel fully embraces being a woman and Makoto ends up impregnating her like the vision showed, but she retains her full healing abilities because her god has a pregnancy fetish.
