When I read it I was figuring that he thought his bro was a "male" the entire time and got proven wrong at the end. And yes while the author had her wearing breast forms it's possible that they were there to "enhance" her... well not as ample bust. Since she was trying to "accentuate" her femininity the breast forms were a good trick. The response of "I'm your boyfriend but with tits" kinda implied that they were always there.

Nope, it's a girl. The uploader is a dickhead that censored page 16 leading to this confusion. She was wearing fake tits over her real ones. Also page 19, she figures out that he thought she was a guy and says "well lets just pretend I'm your boyfriend, but with tits". This manga did initially have a BL tag but was removed once the confusion was cleared up.
