When I read it I was figuring that he thought his bro was a "male" the entire time and got proven wrong at the end. And yes while the author had her wearing breast forms it's possible that they were there to "enhance" her... well not as ample bust. Since she was trying to "accentuate" her femininity the breast forms were a good trick. The response of "I'm your boyfriend but with tits" kinda implied that they were always there.
