Also, people not boil the water? I'm not nure what kind of fantasy but boiling water to make it safe is the go to for many isekai. Unless this one opts for "historical" "realism" so people just drink alcohol all the time

When I was a teen our camp's water got some sewage contamination (was faint enough that a human couldn't smell or taste it). Almost everyone got sick but the symptoms were vomiting and diarrhea, not numbness in the limbs.
It's actually rather common in 3rd world countries, especially where animals bath and water near villages and small towns.Im gonna hurl. If i had to guess its either damage to the capital(?)s sewage system that leaked into the nearby water supply or its intentional.
Boiling water is actually very expensive from an energy standpoint when your primary/only source of fuel is wood, it's why bathing was something reserved for royalty/nobility. I think it depends on the specific source of infection present in the sewage water. Dysentery is caused by one kind of bacteria, while Cholera is another sewage related infection that's caused by a different kind of bacteria. Then there's the deadly poop snails that cause Schistosomiasis, of which coughing is a symptom, that kills anywhere from 5,000 to 200,000 people a year.@NotALewdGuy @Gletscher @ryuumajin
Most likely it's Dysentery. And yes, drinking water contaminated with sewage would cause vomiting, diarrhea, and all sorts of gastrointestinal issues rather then the coughing shown in the chapter.

Conversely in first world countries it can happen when private wells are contaminated by a septic drainage field leaching into the watertable or flooding causing havoc. If you are ever told that water is non-potable, it was likely contaminated with either sewage and pesticides/fertilizers.
