Is it necessarily an 'illness'? Or could it be a mutation / foreign genes that can be benign & give super human abilities, but can sometimes kill the person?

Kind of like a super-transplant. If the body rejects it, you either die or turn into a monster.

I'd think that maybe if you have a core, but die BEFORE catching the disease, you won't become a monster. What Usami described is essentially what Tarao and Mikura had - those black spots which grow and cause pain. It's still very bad if you have to die early not to become a threat afterwards, but that's how I see it.

I'm not sure if that's the case. People who're already in this hospital of his don't seem to be angry or betrayed, while people outside have already proven to be pretty double-faced and manipulative.

For example, Mizuhashi didn't really look dead to me at all. We also didn't actually see any confirmation, so I kinda feel like the group's new leadership either straight up lied or "finished off" their previous leader to push regular people into taking arms.
