Unless it's a delayed reaction or an ass-pull by the author, my bets are on it being not a straight up gender change but instead a "your inner desire, even if you don't fully get it" kind of thing.

Explains why the president basically just got a penis but is still pretty much feminine-looking to everyone else, why Hinata (who was already bordering the line between male and female look-wise) changed genders but not in an extravagant way, that one girl in the library basically looks like a girl except for the penis, etc.

Could be wrong, but it does make sense of why every transformation isn't a straight-up original gender -> opposite gender.

If so, perhaps BOOBS has some kind of immunity/is resistant to it.

...Though that would raise questions as to why her brother was affected, but not her.

Maybe it has to be between two people of the opposite sex? Both she and Hina were technically female when the kiss occurred, unlike every other so far.

EDIT: @phil777 is right. I forgot Prez kissed Hina after she has already been infected when both of them were girls.

I personally doubt that she wasn't a girl in their childhood, with how she reacted to finding that Hina was a guy.
@8mnifarious The "they must be opposite sex" theory is the best one so far, as far as I'm concerned.
It's either that, or she has an immunity, which could also serve as a plot device to help create a vaccine and/or antidote.
