Okay, here's my prediction: Shiori and Hinako are both completely unable to break the promise. As long as Shiori is physically capable of killing Hinako, she will be forced to.

However, that all hinges on Shiori being alive to kill Hinako. Of course, I don't think she can kill herself to nullify the promise. I doubt a binding vow would allow such an easy escape.

And that's where Miko comes in. Whether or not she ends up knowing about the vow, as soon as Shiori ends up going after Hinako, I think Miko's gonna kill Shiori. There might even be an interesting dynamic where Miko finds out about the vow and needs to keep Shiori close enough to kill once the conditions laid out in the vow are met.

Ultimately, my prediction is that Miko will kill Shiori when she goes after Hinako, but will die in turn due to her injuries. Then Hinako will need to figure out what to do from there. It'll be even more fucked if the factor that fulfils the conditions of the vow are Shiori/Miko.

I can’t see this as a positive development…
Hinako has too much to bare on her shoulders right now. The survivor guilt had already tormented her for years, now the guilt for having suicidal tendency appeared only to heavy her mind even more. She chose to live, despite wanting to die for years because Miko was there. However, all it takes for her suicidal tendency to actually resurfaced is a monster who is like ocean. Now that she knows that monster also treasures her, will she able to continue desiring to die, or keep living and being tortured for lifetime, for the sake of people who love her?

Calling it now, the ending will be Hinako consuming some of Shiori's flesh, doubling down on the blood transfer and becoming immortal. They'll be together forever, and Miko too.
