I'm not too surprised, since the artist for this also did
Mizuiro Jidai. I remember there being a panel in there after the MC got her period with her friend bluntly telling her if her flow's that heavy she'll need to change pads more often. (Or something along those lines, it's been a long time since I saw it.) She's always been very direct about stuff like that, even back in the 90s, so I'll bet she had a hand in getting this included in the story.
Can confirm, a developing bosom is very sensitive. Like being constantly bruised and doorframes are the enemy.
And regular clothing feels like sandpaper. Training bras are hella important, even when you're basically flat. Also showers can be the enemy if you have really high water pressure like I did.

I think Eito is simply in love with Uesugi and doesn't care what gender Uesugi is, since he clearly seems to still like them now that they're a girl.
It's an excellent demonstration of narrative priorities that Eito instantly convinces everyone that Uesugi really did get turned into a girl, and no one asks why. "What did mom tell the school?" is a great question that I'm not sure the manga can answer.
I think what happened is that whatever his mom told the school didn't get passed along to the students. The
teachers were expecting Uesugi to be female, his classmates weren't.
