In fairness, leaving a marriage is NOT that easy, especially as a housewife in Japan, doubly especially when it was an arranged marriage. Where are you going to go? You’re not financially independent enough to live by yourself, your parents aren’t going to take you back in because they’re the ones who put you in that position to begin with, and it doesn’t really seem like she has any reliable friends.Husband not paying you enough attention? Too stupid to convey your feeling or simply leave? Just conspire with someone else to effectively drug and rape him while he's unconscious over and over again.

It's unclear what she actually told him, though. In the first chapter when she's thinking about how bad he is at sex she thinks of a few things that would improve it for her but intentionally doesn't say any. All it seems like she told him is that he's under-performing/it's unsatisfying. She repeatedly points out that she can't communicate with him and doesn't have the courage to bring anything up which is why the ease she has "talking" with Gou serves as a contrast.Also it’s mentioned before she HAS tried talking to him, and he shuts her down.
Women read these kinds of stories as "liberating" and sympathetic but it's entirely cruel on their part.

(People explaining why she doesn’t immediately exit the marriage is not the same as applauding her actions! With regards to being uncomfortable, that is absolutely the intent of the author. But my real discomfort is with whether the author is "in the know." Like when someone writes a racist or rapey character sometimes you can't immediately parse whether the author knows their opinions are bad. I assume itagaki does know based on past work, but it just hasn't been fully revealed by this point where the story will go and who and how it will vindicate. I think it’s pretty clear that the situation has spiraled out of her control, and while she’s still a willing participant, it’s doubtful that she could “run away from” Gou all that easily if she changed her mind on him or started to be bothered by anything he did. Hardly a liberating fantasy!
