What a strange comparison to make: Most people do not identify with the perpetrator in the crime novels. NTR on the other hand has one single purpose, for sickos to fantasize about harming other people and getting sexual satisfaction from it.

I don't really see how liking to watch bunch of students getting murdered one by one by serial murderer or a monster, or trapped and tortured by some sicko like in Saw movies, is that much different from liking NTR. Or Saw fine, but Human Centipede bad? There is continuum of suffering in many media of entertainment and art, and it's not that convincing to say that this fictional suffering is fine, while that fictional suffering is bad. Are people even allowed to root for bad guy protagonists, like Bonnie and Clyde, or From Dusk till Dawn, or Overlord LN/anime? I didn't enjoy "From Dusk Till Dawn" since MCs brother was rapist-murderer, and decided against Overlord once I've heard what cruelties MC's side commits later, but it shouldn't mean it's wrong for other people to enjoy it. Or to like adult-high-schooler age-gap romance. We may get into rather prudish territory here, where every story and character is graded on being moral enough to pass.

I'll give you that there's no clear line and yes there's torture-porn like saw which is clearly made for sickos as well. Still I cannot agree tho that there's a line between fiction and reality (in regards to the mindset of those enjoyers) when it is something so clear cut as NTR ( or Vore maybe). The people who can seriously only get off to something like that would do so in RL in a heartbeat if it didn't come with any consequences. The only reason they don't is because they fear repercussion ( - lucky us).
You can't convince me that those who relish the fantasy of causing suffering would earnestly feel bad in the same situation in real life. I don't see how calling them what they are is prudish, I wouldn't ask for any of those mangas to be banned because it is pointless anyways. There are no victims and the mentality of those people won't disappear either if they were to be banned.
