i would've expected some live in nurse or a maid/butler to be there but I do wonder if there's some 'bystander' law or so against just watching someone die and not calling an ambulance versus like watching someone drown (altho hypothetically even if one can swim unless they had lifeguard training idk how many ppl would successfully be able to help someone that way versus calling for someone [then again japan is kinda a 'look the other way' type of society])
