Is this some kinda bizarro universe where neither anal nor contraceptive pills exist or did I miss something?
Japan is still trying to fully process culturally the distinction between prescription medications and recreational drugs. Something you take on the daily but isn't necessary for survival -- contraceptives, antidepressants, ADHD meds, you name it -- exists there in a weird valley.

Yes the pill exists, but it's not used nearly to the extent it is in the rest of the first world.

Ah, just like there's no lactase pills anywhere in Japan, Asians mostly lack the genes to keep producing it past infancy, milk is marketed as super healthy, and somehow there's a ton of meds for stomachaches, huh?

There's quite a difference between, on one hand, dietary supplements and various occasional-use OTC items (and even single-use/short-term doctor-prescribed stuffs) and, on the other hand, long-term prescription meds. It's much easier to get Plan B in Japan than the Pill.
