Of course the sister likes him, because the protagonist is what is called a 'black hole sue'. The world revolves around him. Isekai often involves some measure of wish fulfillment, but I've never seen a case as egregious as this one. Usually there is suffering and/or challenge to balance it out.

One of the girls he likes (he has like eight girlfriends by that point) is from an opposing faction, and thus they can't be together. The game hero's childhood friend. That's the extent of the tension from as far as I've read, which is until the end of the tower dungeon.
