If you've actually read the manga, you'd know that him drinking her medicine was what motivated her into drinking her meds and saving her life. She had long stopped believing that the medicines would cure her and that there wasn't any after effects.

Prez knows that Sorapoyo is super dead set on Myaa, and when he says he cant entretain someone who he knows isnt into him, he isnt really shoting Prez down, if anything hes admiting that he is fighting his atraction to her, but Prez is misreading it. Because while Sora thinks she is after the misterious prince and would never go for him, the reality is that not only he is the prince she knows he is.

At the same time Prez doesnt look like she is willing to cheer Sora into Myaa´s arms, knowing he too has indecent thoughts has only strengthed her resolve.
Myaa better have saved Sorapoyo´s life in the past because there really isnt any rhyme or reason for his atraction, they got no chemistry and she is super stand offish.
Dunno the best boy and best girl in this mango are unquestionable and suffer no rivals.
