Well, people are more than their fetishes. He didn't die "unfulfilled", he died while never properly managing and confronting his fetish, which was apparently a particularly intense one, approaching obsession. Just suppressing a desire usually doesn't end well, you have to actually process it and either fufill or properly contain it. Possibly some mix of the two.
