If I remember correctly, he wasn't even a classmate, per se. Just someone who contributed something for local service. So for him, she was "Patient X who asked for an anime character figure," whereas for her, he was "The totally awesome kid who granted my special request for an anime character figure."

I hadn't thought about it from the time, but yeah, if her style changed this much, and he's already bad with names, then of course he wouldn't remember or connect her with her past self.

No they were classmates. However because she never attended class he never really met her outside of the few hospital visits the class had. He had made a figure for her which had given her the mental strength to endure through her illness. While it was something very important for her to him it wasn't anything special as he was always just giving out stuff he made to others. Her mother knows him as well because of said visits.

She also knows full well how stubborn, selfish, and unreasonable she is being by expecting him to remember it all. She is hopelessly in love with the past version of him and has completely fallen for the current him as well.

Also for some reason the Manga has taken some liberties in buffing her chest size. She's more modest and even has times where she's somewhat dejected about her size.
