its a distorted version of how the society looks down upon those who dont have friends and are lonely. Think abt it, when u are with a group of people are u really on the same wavelength? sometimes being in a group makes one even lonelier. And there's the issue of hikikomoris in japan, the shut-ins. Having fake friends is even worse than being alone. Natsuko was lonely and definitely mentally ill and it's her distorted version of friendships/relationships.
