here i think the author just doesn't seem to know how to do physical comedy in a way that actually lands tbh, it's supposed yo be a joke about her being offended he's calling her old (the setup being that the uncle clearly hates it but doesn't do anything when he hesitates and calls him that, the punchline is that the aunt does not show she hates it but responds with a slap when he says it without hesitation)---they're only used to yuzu saying it since forever, etc.). when you already have a slap scene that was dramatic and the climax for that series of chapters, you can't just do it again so soon but as a joke this time.

After all, she was the one who voiced her opposition to him becoming a member of the family in the 1st place, despite his late father's wish. Because not only he is a bastard child but a commoner as well.
In my eyes, she is not better than the mother that slapped him a few chapters ago.3

edit: also, I honestly thing she slapped him less because he used offensive naming (I mean, he is right. She is his aunt going by family line despite her age) but more of a fact that even mentioning that they are related at all is so offensive to her. I don't know where you get the idea she would slap him regardless, we don't know that much about her.

The slap thing in itself is a joke and it's conveyed through the drawing of the slap. The joke being she is his aunt (obaa-san) but he is not really close to her so it sounds like he is calling a young woman he doesn't know well an old bat (obaa-san). You are reading too much into it, it's a typical japanese joke based on words having different meanings depending on context and the clash of those contexts.
