I'm having a similar problem, except for me it isn't showing a name at all and I can't even reject them.

Similar problem here, can't see person's name and even if it says that I have accepted it it doesn't go away. Reject doesn't work either.

"nameless" ones are simply accounts that have been reported and then deleted iirc. When the V5 launches there will be a way to purge all those requests.

Bot accounts that have been nuked.
Devs will try to remove the requests and notifications.
