@Moth the English doesn't really capture the essence of it, cause Japanese have different words which don't have differentiations in English. the whole ma'am thing in English has a far more respectful tone and can be applied to a younger lady whereas the Japanese term used in the original text "oba-san" is only really meant for older women so the receptionist lady doesn't like it and the others find it funny to see her repeatedly referred to as such...the initial reaction to her being named is because that's the moment they first realize Pamir is being called an old lady (likely she'd much prefer to be called "onee-san" which is used for a younger lady who is still older than the speaker)
@Greyhood - you might have seen this in Sherlock Holmes stories. The narrator/point-of-view character (Dr. Watson) is not the same as main character/hero of the story (Sherlock Holmes).
