i've read ahead with the japanese raws and all i'm going to say is don't waste your time with this manga, you're only going to be angry. massive spoiler ahead, click at your own risk. number 3 is very big spoiler.
1. the series is finished/axed and the ending is super dumb. only 51 chapters
2. MC ends up together with akaneya/iida but then he suddenly has ED when it's time, they still do it but he can only finish by his own hands.
3. anyways some time passes, the sex friend senpai brings iida and her friend to this storage room and has "sex" with both of them off screen. MC sees the senpai bringing iida and friend to a storage room but doesn't go and save her but rather goes to the bathroom and starts to fantasize the senpai f***ing iida and tugs one out like the cuck he is. he later then confronts iida about whether she had sex with the senpai or not, and iida says she did. so mc breaks up with her. and then time skip 6 years later and apparently iida actually never had sex with the senpai, and she actually 'loves' mc (mc is special to her that's what it literally said) but MC doesn't actually like the 'girlfriend' iida but rather a broken iida that has sex with randoms and raped. that's pretty much the end, nothing else.

Iida never had a threesome with the senpai; yes. The panels with her having sex were MC imagining it because he has a cuckold fetish. Same with the other men (like the shopkeeper 'cheating' with her on his wife or the sugar daddy dating bits). Iida is lying to the MC to satisfy his cuckold fetish because she has understood he can't function properly and have a different relationship with her. The senpai story is the FIRST story of her starting to lie to the MC to satisfy his cuck festish.

The same applies to the shopkeeper episode where in reality Iida stays to be with his wife and him getting some food but lies to MC that she slept with the shopkeeper who was frustrated with his wife. In reality, she never had sex with these men pictures in the last chapters; it was all a lie. She comes up with different 'scenarios' so the MC keeps his cuck fetish active and 'interesting'; basically she's the storyteller and creative writer for cuck MC's wishful fantasy.

Yeah, I feel the whole series is a bit of a rage-bait and could have been executed way better. However, Japanese readers really like this kind of storytelling/scenario, at least way more compared to their Western counterparts. That's why the later volumes of Karami have 4.1 to 4.3/5 on JP sites.

Akaneya doesn't get a bad ending; there's an extra mini-chapter that shows her finally moving on, going on a mini-date sort of thing with a guy.

She did have sex in the beginning chapters of the story, but as the story progresses (and her relations with the cuck MC) she realizes she doesn't have to force herself to ACTUALLY do it. She can just tell stories and thus satisfy his fetish. It's the culmination of her feelings towards him.
