I think it's quite clear: Onii-chan is everything to her. Her love for him is her life's purpose. She loves him more than life itself.
After enduring much hardship, she finally won his affection - or so she thought.
Just when things were finally starting to look up, he delivers the most shocking betrayal.

If her beloved Onii-chan does not even care for her enough to go a single day without cheating on her, why even live? He was her whole world and he abandoned her like trash. He only got her hopes up to hurt her even more.

Even if this isn't what's really happening and it's all just a misunderstanding, this is how it feels to her. He hasn't attempted to explain his actions and/or clear any misunderstandings. He just left her with the impression that he doesn't value her enough to commit to her for even a single day.

Also of note is that only now that she's broken, does she reliably receive his care all day. Even if her heart does heal and she comes out of the depression, she may just have to keep acting this way for a while so her Onii-chan doesn't stray again. He really needs to do something for her to show her that she's the most important person in his life now and earn back her trust.

Even if they can't get married, he should give her a ring and impregnate her with his child to demonstrate that he's serious enough about her this time to give her that role in his life.

There’s two by the same author. In she used to be cool the big sister is just not a very functional person anymore because of depression and in we used to be happy the big sister is suicidal.
