This one seems less shit stormy. "Things would be less complicated if I was born heteronormative" is a pretty straightforward one. It's not exactly disphoria. It's just wishing things were simpler.
I suggest looking up “heteronormative“, because this is kind of the opposite of what anyone in this manga is.

Which is my point. this manga isn't heteronormative. Hiura was saying this would be less complicated if it was just a boy meets girl type thing. Based on the next chapter especially, he seems like he thought midou was straight and he wasn't sure he was "girl" enough for him to like him

i really don't like the direction this has gone in with Hiura constantly contradicting himself about whether he crossdresses for himself or Midou, whether he's happy as a boy or wishes he was a girl etc... it feels less like any kind of internal struggle and more like sloppy writing
he started off crossdressing for midou, but slowly relized that wearing beautiful things will make him happy. he knew that he's a boy, but lose his sense of self during a moment of desperate and frustration. messing up your initial intention with what you really do feel like, that's what i would call internal struggle at its finest.

I think it's very well written to be accessible to the broadest demographic of readers. How many readers want a cute girlfriend like Hiura? Him being gay and being a boy is just a quirk... that's my cope! 😍😍

In rare moments of objectivity (which I push away as fast as I can), I see that this might not be the most faithful reading, and hope it doesn't come across as gay erasure, which is not my intent. I am just reading for enjoyment after all and like to view Hiura differently from chapter to chapter as the setting changes, usually as a cute girl, so it helps for me that he's written a bit that way, at least it's not so much of a stretch to read it that way (more coping). Since this series does not have a suggestive tag so I doubt there's going to be some genital jousting that will kill the suspension of disbelief.

I am also cheering for all you others who might want to see him differently!
