The leader could be gay, or Trans. It wouldn't be the first time that Japan has purposely made a villain LGBTQ+Ultra.

If they did, they would design the character that way to properly describe him.

The way that dark elf is designed doesn't in anyway hint towards him being Gay or Trans.

Because normally when a Man is designed as a Gay in Manga, they have a more calm feminine expression, or they have no expression at all and only show interest when a man they like appears (like Talhand from Mushoku Tensei).

And if they are Trans, then they are usually shown cross-dressing or wear highly risque clothes, more commonly BDSM styled.

As for this Manga, as far as i understood from seeing the raw, there is no hint towards him being a gay, or even calling himself as a Lady.
There term used with his name is the suffix "-sama" which is gender neutral in Japanese. But when translated to English, it could either by "Lord/Lady" or "Your Highness" or similar terms.
And as Japanese language is heavily Contextual based, you have to try and stick with gender neutral terms until confirmed.

Not sure about your source, but the Light Novel illustration does list that guy as Idea.

Also, the Manga raw just mentions the name as "Idea-sama (イデア様)", with no specific gender term.

While "-sama" suffix is used to note any gender in power or in higher position/status.

But when gender is not known, its always better to use either gender-neutral term or use term which can be used as gender neutral even-though they aren't (like, while "Lord" is a masculine word, but it has sometime been used to describe Queens as well).

And based on what i noticed in the Manga raw, its clear that the Translator made an error, and translated him as a lady.

I thought the manga had talked about "Lady Idea" previously, but maybe it was just another mistake.

I checked Manga raws, and everywhere its always Idea-sama, which technically could be translated as Lady Idea as well, assuming that you know the characters gender already.

Japanese language is highly contextual.
Many of the terms are bi-gender, aka., can by used for both male and female (which is why Machine translation always make error with gender identification with Japanese).

So, its important to keep in mind to always cross-reference the scene and the contents in the scene while translating.
If the gender is unknown, then its better to use a more gender-neutral term instead.

Male/Female terms which can also work for opposite gender would also work.
Like for example "Lord".
While, its generally a male term, it has been historically also used for female rulers as well.
Though that's not the case with Lady, which is always female specific.
