Fen Shen started taking customers because her getting pregnant with MaoMao ruined her career. She ended up getting syphilis, which is not surprising for prostitutes in the Red District. She is the one without a nose and full of sores whom MaoMao is shown to take care of. Since her disease was not treated in the beginning, she eventually died.

The novel version is a bit more blunt about it and doesn't imply it like in this version. Not sure if it will be expanded on here as well but in the novel version as far as I remember Feng didnt get a disease from taking customers but went mad from love thinking that she was abandoned, and at one point tried to kill mao mao blaming her
