He dresses like a girl cause of Trauma and his mother, he's taking the place of the more loved sister and they even sad that the male died, not the female
He keeps dressing up like one cause 1 it's a rabbit hole, 2 his mother would retaliate, and 3 without outside interference, this will keep happening, heck the mother already written off the male being dead, so this is mostly because of his mother's mental state
His parents keep doing this cause the mother's mental state, this is a way she copes, and she's unafraid of what others think for 2 reasons, she will wholeheartedly reject it all and believe her son is dead and only her daughter remains cause trauma, and 2 cause people already think the male child is dead. (I think she's the only parent alive, or they are just very busy or hoping it will fix overtime)
She keeps it to herself cause like you said before rumors, that and who's willing to open up to people about inner turmoil besides close friends and family, and even then some people just don't keeping it bottled to themselves cause they believe they can stubbornly fix the situation by themselves.
And people aren't gossiping about it cause 1 low blow and you would be scorned, 2 they are fairly influential, and 3, how could they know when everyone believes that the male child is dead and the female remains?
