Because it's a normal looking creature in a room of horrors, i believe it's related to the uncanny liminal effect, aka where something normal feels far out of place and not supposed to be there.a simple looking man is felt so disturbing in each of its appearances.

I agree that it's a higher ranked spirit, but I think the "creepy uncle" appearance is just a lure to trick people with "the sight" into interacting with it.I disagree. Miko, on the other hand, saw it as something unique. Not just a creepy old man but one with a scythe to boot. To me that suggests Miko's strong enough to see at least a bit of this entity's real nature (he's probably a shinigami, a death god) while her father couldn't.
I don't remember what chapter that was so I can't check, but in 31, she definitely couldn't see it. She could infer information about it from its effect on nearby spirits, but had to rely on Yuria to actually see it and tell her how it looks.
