I asked the same question before, SachelBagman is kind enough to tell me that it's normal for Japanese to address a stranger who is not visibly older adult or a small child -- Onii/Onee. If they looked elderly they'd be calling each other Ojii-san and Obaa-san despite not blood related for being courtesy.

Also, the famouse Kanade I know making the name hard to associated with maternal type...

They never cleared it up with a conversation on who's older, so they both default to the polite term you use to someone you've just met or aren't close with even after all this time. I think it's just a running gag at this point.
