It's not about the ear cleaning specifically, it's about the intimacy and trust involved.
Are you sure you can't recall any instance as a child of a parent cleaning your ears, washing your hair, clipping your nails or something similar? It's a comforting feeling.

I completely agree that ecchi series presenting ear cleaning as a pleasurable act is a bit weird though.

I stepped away for several reasons, but seeing that no one picked this up after 2 months, I decided to pick this up again and will probably try to catch up to the raws, unless someone else wants to take over the translation of this manga that is (please? pretty please? anyone?)

I think one reason is that most east Asians have the dry earwax gene so it is easier to remove the dry earwax than wet earwax. I think this also shows up when you see a character sticking their pinky in their ear and then blowing off the earwax on it.

I don't know how the real thing feels (and probably never will since I trust doctors who say I shouldn't insert them nilly willy and my hearing already isn't that good), but the ASMR version of them is usually pretty nice. Here's a short example, though kinda goofy.

I don't know how the real thing feels (and probably never will since I trust doctors who say I shouldn't insert them nilly willy and my hearing already isn't that good), but the ASMR version of them is usually pretty nice. Here's a short example, though kinda goofy.

Asians generally have dry earwax that is fairly comfortable to remove, so the process is roughly as gentle as combing straight hair. But guys tend to have short hair, so this is probably the most common physically intimate interaction that involves love and care.

If guys more commonly had long hair, hair-combing would likely be much more popular and show up in manga and other media though.

In my case, it got sucked out. Not wholly pleasant, but was better than using tools.

My issue was not earwax though, seems to be TMJ.
