With how things are evolving, I'm pretty sure Rio cheats with Mahoro's husband just because she was bored and frustrated in her couple's life. With how her husband was presented, he seems like the type of person who is always busy with work or something else.
I think it was also the case with Rei but in his case he is just a asshole. He just wanted to fuck another woman more pretty than her wife.

I think things play like that: one day Rio and Rei meet up randomly, when they were chatting they found out the two of them were bored and frustrated in the married life so they decide to meet up occasionally to fuck.
The opportunity to cheat was there and they did it.
Or they could have been exes or sex friends before they were even married.

Another thing, Rio seems so fake to me, like she put a facade of someone who is kind to friends but in reality she feels superior to them.

With his age it seems like he might be a snarky teen as opposed to someone being cunning/opportunistic. I don't like the "Mahoro is an unreliable narrator" theories, but in this case we do really only see the friend group's dynamics solely from how she remembers them. In chapter 9 it's implied that Momo already knows a bit more about what Rio is truly like. It is possible that Rio never even had much of a facade and was awful the whole time, we just don't see it that way because Mahoro's memories are tinted by her own feelings of friendship.
