She is a serial killer. The killing doesn't have to bring justice to anyone, she only needs to think the person deserved it (by being a non-repentant murderer).

There's no evidence against him so he will pass away with no regrets and clear reputation, but if his past gets revealed, which is now more likely to happen now that he's relapsing into his memories, he will then istead die on a prison hospital bed disavowed and hated by his family. One of Spica's kinks is that she wants to make her "victims" feel regret in their dying moments.

if anything, I think this was the most foreshadowed twist so far. There was frequent visual emphasis or passing mention of phone cords, and an uneasy atmosphere around the neighbor at the start.

...Well, not that I quite expected this specific reveal. Something about the neighbor and cords, sure, but it turns out she just... kinked too hard.
