Yoshida understands it only if its illustrated in a kiddie way. Mako knows her well and the stuff the likes but others are confused probably because they thought that Mako would draw it realisticaly hence her high health test score. I hope this helps.

Second part of the draw i "understand" this:
The bear throwing the seed implies certain abuses that happen in Japan (think of the first encounter with Hannibal in Silence of the Lambs) or a sexual practice. Or Mako explains that the bear tries to forcefully put his seed in the bunny, and Yoshida never must allow that.
In the left is explaining lesbianism, but i don get the meaning of the naruto flower...

And the flower is a common symbol or sign used by teachers in Japan to grade a work, meaning it was an excelent job or something like that
