That was pretty much my takeaway too. The artist is testing how far they can push explicit content while poking fun at the double standard between female and male nudity. By keeping Hiki's gender "ambiguous," they can show off her body under the pretense that those might be male nipples. It reminds me of mg85, where a teacup cap stands in for Tatsuno's areola. Technically it's not nudity, but come on.
