It's definitely not a good ending but for some reason feels so satisfying to see her achieve some sort of "payback" for Emily not feeling the same way. There are tons of gl plots where one is marrying a man and all the other does is cry about it, which makes this so much more fun to see.

Not sure about stature (I'm assuming that the real Kaoru and Lily didn't touch much when they met the first time though), but it was explained in the first chapter that Lily believes "Kaoru's" voice is higher because of his tuberculosis, which was what actually caused the real one to die

I'd say it just visually represents how desperate/determined she is to finish it off (hence the multiple methods shown in the picture), but is constantly stopped by the angel trying to persuade her into picking reincarnation
