It's not that it was a dress, I think all baby clothes back then were like that instead of onesies. It's just more difficult to tell a boy and girl apart if they're so young, and since this is a manga, hair has been added (suggesting she's not as much of a newborn than she is) instead of a wrinkly baby.

Babies being born bald is actually pretty rare. The norm is for babies to have a full head of hair, how thick the hair is depends on the baby and it's not weird for babies to go bald after awhile and grow hair that is a different color. In fact one of my baby cousins was born with a thick head of hair that would make a lot of older balding men jealous
