I do not believe it has been stated this world does NOT have cosmetic surgery or similar available.

I hate MC who isn't confident too.
But I think MC's low esteem actually makes sense in this manga.

In a world where a skill is all about defeating monsters and clearing dungeon, he got a skill that is useless on that front. It's normal to feel discouraged in that situation.

And unlike doctors/technicians who actually study cosmetic surgery for years, he only learned about his skill overnight. It wasn't something that he trained himself.
He had no idea about the possibilities of cosmetic surgery before this.
Only after his gf shed the light for him that he knew there are demands for his skills.

It has only been a month, and this new girl is only his fourth patient. It will take time for him to be comfortable.

Problem is, MC is still a High School Student (considering in Chapter 1, it was mentioned that he just graduated from Middle School, that means he is currently 16+), so he is restricted to work related to only the Guild.
For any other type of work, he has to become 18+ before doing anything.
Also, all these interaction with the girls is basically making him "gain confidence" to begin with.
I mean, his Skill "Boob Corrector" is practically exclusive for usage on Women, and unless he is used to Women body in general it would be hard for him to make a career out of his Job once he atleast graduate from High School.

As for "cosmetic surgery or similar available", well the problem is that it depends on person to person on whether they could take the procedure or not.
Like you said, doctors/technicians make a lot of money with cosmetic alternation/surgery, which means those are costly procedures. So, it's not like everyone would be able to afford them.
Then there is also the fact that, those who had worked as Adventurer end up having higher defense stats, which makes it hard to perform medical procedure on their bodies. I mean, just take the example of the previous girl with small hair on areola issue. Why was she so bothered by it when she could have practically shaved or waxed them. It's because she had high defense stat, which made stuff like those difficult to do (i mean, her hair probably are as strong as a small needle while still being flexible).

Cosmetic surgery doesn't exist in this world. Neither does hair removal. The only possible solution for these issues is Takeru's boob massages. He's doing them a favor.

Cosmetic surgery probably exists, but not everyone can afford them, especially High School girls.
And depending on the damage it might not even be possible, like in the case of the girl who lost her right-boobs (while she is currently in 1st year College, but her accident happened 3 years back, aka., when she was in either 1st or 2nd year High School).

As for hair removal, that's basic and does exist (I mean none of the other girls had asked MC about it).
The only reason Knight girl asked about is because her situation is a bit different.

First of all, none of the hair removal techniques are a permanent solution. Depending on the the method, one could get a long term relief, but they costs more as well. But eventually it would grow back.

Secondly, Knight girl is an Adventurer, so her stats are quite high, especially her defence stats. As Mindread girl mentioned before, Knight girl is so defensively strong that a car could hit her and it would be the car that gets damaged.
This applies to every part of her body, including her hairs, aka., her hairs are stronger than needles while also being flexible.

The issue with her is that the hair on her areola are the prickly type, aka., hard and painful when the get rubbed. Shaving them is useless, and because of her high defence stats, any hair removal technique is going to cost more than normal that to it would only provide relief for a short duration. Though she is an Adventurer, meaning she earns a lot more than a normal person of her age, but that doesn't mean she has to waste it on some costly hair removal product/technique which isn't hundred percent guaranteed to work.
