But I assume there's no other save point? Every time he dies, he started back at lv.1 inside this dungeon again?

If so, the stake is becoming quite high that I'm not sure if another death after this point onward is still an option that us as a reader have to fear the author could still pull off.

Not only did he beat final boss, we caught up to the final chapter. I love you @LonelyDarlingUwU.

& it better not be Ageha preventing him from leaving the dungeon
There two things that stand out to me about your comment:
  1. The title: My Yandere Girlfriend Lurks in the Dungeon and Kills Me Over and Over Again
  2. I think the author spoiled the story in the ORIGINAL cover art that is no longer being used.
  3. COVER ART SPOILER (click image to enlarge)

As you can see... there are 3 girls on this cover. Two of them are Yanderes that prevent him from leaving the dungeon. One of the girls is his childhood friend Namia.

    Shitty Speculation:

The room with the three teleports each lead to a Yandere girl. The sword is being alluded to probably being a girl. What if the sword is Namia or something?

I’m still confused how the hero freed herself and and started killing him for no reason.
You have seen enough pieces to have an idea. I mean she gave him "Save and Reset" right?

A slightly spoiler-y hint: You think that the person who granted him the ability to manipulate time would be unable to do so herself?

Just how do you think these savepoints have been working? Clearly some things like the sword and hero are "remembering" things that happened before... perhaps they don't get affected by the reloads, hm? If thats the case... ...perhaps shes already free.

What, the carapace has directional defence? How was he unable to cut it from the outside, but able to cut it from the inside? Like, sure the meat is unprotected, but the carapace is still there regardless from where he cuts from.
:questionblob:
If the part of the carapace was simply blown of the body there could be an explanation for that being possible.
But I agree the piece of carapace clearly being punctured through makes no sense.

There are only 2 possible explanations for that:
1. Most likely simply plot armor, the author and artist never really thought about it.
2. The centipede reinforced his carapace with magic power or so - like self enhancement or self strengthening.
Then upon receiving a heavy injury to its insides the centipede wasn't able to continue with that self enhancement due to losing concentration because of the pain or/and its flow of magic energy being disrupted due the heavy injury.
Without that self enhancement the carapace was then weakened enough for the sword to puncture it.
