Hot take here: I think this is all okay. It could be worse since I actually envisioned the MC finding his little sister in someone else's arms at some point. So he gives in to a beautiful nice girl he knows well enough that relentlessly attacks him. How can a young hormonal high school boy not submit eventually? Sure, he's got some vague romantic mutual understanding with, oh right, HIS SISTER. I'm not about to rant about incest not being okay, because incest is wincest as far as fiction goes. But real high school boys aren't exactly known for their faithfulness and ability to resist temptation.

Besides, it's not like what he did is a sin. Akiho seems to be a genuinely good girl, even if she's being "bad."

I think the implication is she's using him just as much and maybe more than he's using her.

I’m not so sure, the best way to get a lead character hated is for him to chose a girl and then betray her, ESPECIALLY if it’s the main female lead. Yes there may be a few folks who quibble but as a rule folks only get annoyed when the MC is wishywashy over an undecided harem or with the FemC. Readers want their MC to not be spineless and what MC is doing currently is the single most spineless move I can think of in that situation, where as if he held out and turned her down decisively he’d probably get showered with “Chad!” And “Thot Slayer!”

I'm not sure I agree. Was he indecisive? Heck, no, he did decide here. Was he spineless? I'd say no since spinelessness can also be not having enough willpower to make any decision, whatever that decision is. And he made a choice. One thing is clear; this MC was not made as a role model, and if you accept that, it's easier to digest this story. It happens ALL the time: when any form of relationship is long distance, it tends to die. Painting a person as some form of hero by staying faithful despite that distance without any form of actual commitment from both is incredibly naive, especially counting the fact that these characters are literal children.

Here's the thing I don't get about that...

MC and Guitar Girl are both in the same grade, high school first years. In chapter 3, they talk about when their birthdays are. His is in April (the translation says August but the calendar shows April, so I assume the translation is wrong) which would be the beginning of the school year and hers is in October. That's only 6 months! How the hell could they be siblings? If hers was like February or March at the end of the school year, it might be possible, but not October.
